Long-term care has consolidated heavily since 2021, so several companies that appear on older lists of top nursing home operators no longer trade in the same form. Genesis HealthCare, for years the largest operator in the country, filed for Chapter 11 in July 2025 and the sale of roughly 175 of its facilities to NewGen Health was approved in court in January 2026. SavaSeniorCare wound down in early 2023, Consulate Health Care broke up following a 2021 bankruptcy, and ProMedica Senior Care left skilled nursing in 2022. Avamere announced its own exit from skilled nursing in 2026. Facility counts move constantly as portfolios change hands, so none are quoted here; treat this table as an orientation to who operates at scale rather than as a ranking.
